CERTIFICATE OF CAUSE OF DEATH (CCOD)

Obtain Certificate from the Attending Doctor

REGISTERING THE DEATH

At Government of Government-restructured Hospitals

• Register at the respective hospital's Medical Records Office (MRO)

At Private Hospitals or Nursing Homes or Outside Office Hours

• Register at Registry of Birth & Death (RDB), or at any Police Station/Post

Documents required for registration:

• CCOD issued by attending Doctor
• Identity Card of the Deceased

Death Certificate will be issued in duplicate, together with a Burial or Cremation Permit

• Original retained by the Police/Mro
• Duplicate issued to the family/informant
• No fee is payable for registration of death

  DEATH AT HOSPITAL

• Upon the certification of the cause of death, call AMAZING GRACE at 62522166 (24-Hours service) immediately
• AMAZING GRACE staff will collect the body as soon as possible
• Contact your Pastor/leader to arrange the Wake and Funeral Services
• Decide on the form of disposition - i.e Cremation or Burial
• Casket Package, Chapels, Tents, Flowers, Buses, Etc

DEATH AT HOME

• Death must be certified by Family Physician, who will issue the CCOD
• If you cannot find a Doctor to certify the death, AMAZING GRACE can assist in recommending one for you
• Upon certification of death, call AMAZING GRACE at 62522166 immediately
• AMAZING GRACE staff will collect the body from your hoyse as soon as possible
• Register the death at any Police Station/Post or at the Registry of Birth & Death (ROBD)
• Follow remaining steps of "DEATH AT HOSPITAL"

DEATH OUTSIDE SINGAPORE

• If a death occurs in a foreign country, a Death Certificate must be obtained from that country
• The Singapore Embassy/ Representative office in that country will issue the repatriation documents
• A Consignor (usually a funeral home in the exporting country) will assist in the repatriation arrangements
• AMAZING GRACE can be appointed the Consignee in Singapore to receive the remains and to provide all the subsequent arrangements and services

POINTS TO NOTE

• If the attending Doctor suspects that the death is due to unnatural causes, the death will be classified as Coroner's case
• Coroner's review will be performed before a CCOD can be issued
• All Coroner's reviews are performed at the Centre for Forensic Medicine (CFM) mortuary (SGH block 9)
• Burial fee of $940 and Cremation fee of $100 are payable to the NEA (NATIONAL ENVIRONMENT AGENCY)S
